Position Summary

 

The Health Care Services Coordinator works to provide Charlevoix County residents and seniors access to necessary programs and services in conjunction with the Commission on Aging. The Health Care Services Coordinator regularly assesses client needs, helps coordinate care and programming through phone calls, home visits, and in-person meetings. The Health Care Services Coordinator works with the Health Care Services Director and reports directly to the Executive Director of the Commission on Aging.

 

Primary Duties & Responsibilities:

 

  • Addresses the physiological, psychological, and socio-cultural needs of Charlevoix County seniors.
  • Maintains records of client’s care, condition, progress, or problems to report, and discusses observations with the Health Care Services Director and/or Executive Director.
  • Assists with the Home Delivered Meal Program: Calls, client set up, assessments, reassessments, and client removal.
  • Establishes and maintains constructive and cooperative interpersonal relationships.
  • Develops, designs and creates new ideas, systems, or approaches for clients and for the Commission on Aging.
  • Travels to client’s home for initial assessments.
  • Coordinates and schedules In-Home assessments and reassessments.
  • Re-assess clients every 6 months and update plan of care.
  • Updates and maintains assessments and re-assessments using the Carevantage software program.
  • Coordinates the Home Health Care Services Reimbursement Program for Charlevoix County.
  • Coordinates health care presentations in all of the Charlevoix County Senior Centers with the Site Coordinators and Assistant Director.
  • Follows all universal precautions for the protection and safety of clients and staff.
  • Participates in regular Charlevoix County Commission on Aging team meetings
  • Participates in Quality Assurance studies as needed to improve the Health Care Services.
  • Speaks in public to various agencies in Charlevoix County.
  • Maintains client confidentiality at all times.
  • Assists the Director of Health Care Services as needed.

Job Location & Working Conditions:

Position Summary
The Senior Services & Volunteer Facilitator will function as the volunteer coordinator as well as the back-up transportation resource for the Home Delivered Meal program, cook/kitchen helper and back up Site Coordinator on behalf of the Commission on Aging and reports to the Assistant Director of the Commission on Aging.

Key Areas of Responsibility include but are not limited to:

Volunteer Facilitator

  • Oversee the Charlevoix County Volunteer Program.
  • Recruiting new volunteers. This would include public speaking appearances to promote the volunteer program.
  • Assisting new volunteers through the steps to become an official Charlevoix County COA volunteer including background checks.
  • Scheduling volunteers each month. Providing a volunteer schedule to the Assistant Director each month by the last week of the previous month.
  • Training new volunteers.
  • Providing two volunteer trainings per year for each volunteer position in coordination with the Assistant Director.
  • Maintaining all volunteer records.
  • Back-up HDM driver as needed for uncovered shifts or call-ins.
  • Compiling volunteer hours report each month for the COA Executive Director.
  • Planning one volunteer appreciation event per year in coordination with the Assistant Director and Executive Director.
  • Assistant Director and /or Director will complete the evaluations.

MMAP Facilitator

  • Become MMAP Certified. Facilitate a MMAP Program at the COA.
  • Recruit volunteers to become MMAP certified and assist the COA with a robust MMAP Program.

Back-up Cook & Kitchen Assistant

  • Be dependable with work schedule as assigned. Assist with Special Events.
  • Attend staff meetings, Cook’s training, CPR, and other in-service training as appropriate.
  • Must be able to communicate effectively with senior citizens, co-workers, and office staff.
  • Assist in the preparation and packaging the Home Delivered Meals for delivery daily.
  • Make sure all stock is rotated with older to the front and newer to the back as orders come in.
  • Assist with greeting, supervision and safety of senior participants at the meal site.
  • Assist in keeping the kitchen and equipment clean and in good working condition.
  • Assist with setting up and cleaning of the salad bars.
  • Assist with
  • Wrapping Bread; Put fruit and soup in cups; Wrap Silverware; Baking cookies, etc.
  • Picking up food or supplies as necessary. Putting food orders away as necessary.
  • Washing dishes as necessary.
  • Report any malfunctioning equipment to the Food Services Manager or Assistant Director immediately.
  • Assist Cooks and the Food Services Manager with inventory of food supplies on a regular basis. This will be done by Cooks/Chefs on a semi-annual basis and by the Food Service Manager on an alternating semi-annual basis for a total of quarterly inventories. Inventory includes Pantry Inventory; Refrigerator Inventory; Freezer Inventory and Specialty Inventory.
  • Demonstrate proper hand washing techniques, knowledge of universal precautions and clean food handling.
  • Become Serve Safe certified.
  • Assist with any other tasks asked for by the Food Service Manager, Assistant Director and /or Director to make sure meals are provided safely.

Back-up Site Coordinator

  • Assist as back up when there is no Site Coordinator on site as meal host and activity facilitator.
  • Assist with meal site statistics required for Commission on Aging reimbursement, accurate meal counts, temperature logs, make change, process deposits.
  • Assist with cleaning and safety/cleanliness checks of senior center to ensure proper functioning of the senior center.

The Antrim Charlevoix Kalkaska Association of REALTORS® seeks a self-motivated and dependable individual with a high standard of ethics and professionalism for the position of Association Executive Officer (EO). The association is located in Charlevoix, Michigan and serves approximately 200 members.

Summary of Position

Serves the membership of the Antrim Charlevoix Kalkaska Association of REALTORS (ACKAR) and is responsible directly to the President and Board of Directors for the effective conduct of the affairs of the association. Recommends and participates in formation of ACKAR mission, goals, objectives, and related policies. Within that framework, plans, organizes, coordinates, and directs the programs and activities of the association. Is a liaison between all standing committees and the President.

 

Duties and Responsibilities of the Executive Officer

  • Administer policies of the local (ACKAR), state (Michigan Realtors) and National Association of Realtors and participate in the formulation of Board Policy as requested.
  • Assist, serve and cooperate with the Board President, Officers and Directors and Committees to insure that all are fully informed on the conditions and operations of the association factors influencing them.
  • Attend all meetings of the Board of Directors, assist and serve all Board Committees.
  • Prepare agenda for all meetings, keep accurate records and minutes.
  • Work with Finance Committee; operate within an annual budget and provide full financial reports to Directors. Insure that all funds and physical assets of the Board are appropriately safeguarded and administered.
  • Oversee and manage the operation of the Multiple Listing System (MLS) and enforce the rules and regulations established.
  • Update Bylaws and MLS Rules and Regulations for National Association of Realtors compliance and approval.

  • Assist leadership in planning, drafting and executing communication to the general membership.
  • Facilitate Professional Standards administration which includes processing complaints.

  • Create content and manage both public and member websites for association using various software programs.

Pay Range: Full time $16.81 to start per Hour with Benefits & a county provided vehicle.

 

OVERVIEW of Position and Duties:

Charlevoix County Commission on Aging is seeking a full time Certified Nursing Aide/Homemaker.  This position is Monday through Friday, between the hours of 7:30a – 4:30p for up to 40 hours a week.  No Weekends, no evenings.  This position is responsible for assisting aging adults in their home environment with activities of daily living.  Routine duties include but are not limited to bathing, personal care, dressing, transferring, light housekeeping, laundry, light meal preparation and engaging in activities to assist with isolation and loneliness.  The right person needs to love to work with seniors, on an independent basis under the supervision of a Registered Nurse.  They should be creative, positive, outgoing, dependable, flexible.  Experience is preferred.

MANDATORY Requirements:

High School Diploma

Proof of current C.N.A (Certified Nurse Aide) registration within the State of Michigan or LPN Licensure.

Be able to pass a skills competency exam administered by a Registered Nurse.

Must possess the physical abilities to lift a maximum of 50 pounds and be able to transfer items to and from vehicles.

Strong positive communication skills

Strong time management skills

 

Environment:

Home based environment within Charlevoix County, using a county provided vehicle.
